The launch of the first volume of Clínicas de Gastroenterología de México in 2025 was received with great enthusiasm by the national gastroenterology community. Its acceptance, particularly among the members of the Mexican Association of Gastroenterology, confirmed the value of having a publication that integrated, in a structured, rigorous, and up-to-date manner, the most important topics in our specialty. This initial success not only validates the editorial effort undertaken but also establishes a clear commitment: to maintain quality, consolidate the project, and ensure its continuity as an academic reference in Mexico and Latin America.

In 2026, aware of this challenge, the editors have planned a new series of issues that seek, in addition to providing continuity to the project, to raise the level of analysis and depth of the content. Throughout this volume, four issues will be published addressing topics of high clinical and scientific relevance, with an integrative vision that combines current evidence, clinical experience, and innovative perspectives.

The present issue, entitled “Acid-peptic disease: current concepts,” offers a comprehensive review of one of the pillars of clinical gastroenterology. Its articles address fundamental aspects ranging from epidemiology and contemporary risk factors to the pathophysiological mechanisms that explain the delicate balance between aggressive and defensive factors of the gastrointestinal mucosa. Furthermore, specific clinical conditions are reviewed, such as erosive gastritis and duodenitis, ulcers associated with the use of nonsteroidal anti-inflammatory drugs, and complications in critically ill patients, highlighting their impact on daily practice.

Articles dedicated to contemporary diagnosis are also included, where traditional tools such as endoscopy are integrated with recent technological advances, including artificial intelligence and new biomarkers, as well as the updated approach to bleeding from peptic ulcer through evidence-based algorithms. In addition, the main complications of the disease are discussed, such as perforation, obstruction, and possible progression to malignancy.

A particularly relevant aspect of this issue is the inclusion of innovative therapeutic approaches. Beyond classic acid suppression, new pharmacological alternatives are explored, including agents such as rebamipide, which represent an evolution in the understanding and management of acid-peptic disease by acting on the mucosal defense mechanisms. This approach reflects a paradigm shift toward more comprehensive and personalized strategies.

Finally, hygienic-dietary measures are not neglected, whose role continues to be fundamental both in the prevention and control of this pathology, reminding us that optimal patient management requires a multidimensional vision.

With this second volume of Clínicas de Gastroenterología de México, we reaffirm our commitment to high-quality continuing medical education. We hope that the present issue will be of practical and academic utility for gastroenterologists, internists, and all professionals interested in diseases of the digestive system.
